COVID-19 Screening & Consent

Please read the COVID-19 Screening &  Consent form (2 pages) prior to scheduling an appointment.

1. You can download and sign the copy electronically and send it to me by email.

2. You can send me an email with a statement  declaring that you have read the document in its entirety and agree to sign the paper document when you come in for your appointment.

Please note that if you do not read  this document prior to coming in, the reading and signing will occur  during  the one hour scheduled treatment time.

3.  If  after  reading  the  COVID-19  screening  and  consent  form  you  do  not  wish  to  sign  it,  then  you  may  choose  instead  to  make  an  appointment  at  a  later  date  once the  guidelines  and  directives  relating  to  the  COVID-19  pandemic  are  updated.
